How to add references in Gemini (Part 2): Gem and prompt released
In my previous article, I introduced a prompt to get Gemini to provide accurate references.
However, users have pointed out issues with Gemini 3, such as failures to retain context (input content), and in my own use, I felt its search accuracy was lower than that of previous models.
🔍 Changes in AI model performance on long-text tasks: A comparative analysis of GPT-5.2 Thinking and Gemini 3note.comTo compensate for this search accuracy, I created a Gem configured with custom instructions.
What is a Gem?
Gems are a custom assistant feature available in Gemini Advanced. By setting instructions in advance, you can make Gemini behave consistently without typing out a prompt every time.
Clinical Evidence Mapper (Gem version)

Access it via the link below.
https://gemini.google.com/gem/13Z6Wa0VZe6UH7O-I8vmDBZ5Rt6xNTUag
Please run it in "Thinking" or "Pro" mode. When you enter a clinical question into this Gem, it returns an answer in the following order.
I have tuned the Gem to cover the following content in each section.
1. Clinical background and question
First, it outlines the definition of the disease or condition, its epidemiology (who it affects and how often), pathophysiology, and clinical significance (what happens if left untreated). It then frames the clinical question to be answered in PICO format (Population, Intervention, Comparison, Outcome).
2. What is known with certainty
It presents findings consistently shown in SRs/MAs, along with effect sizes and confidence intervals. It also includes a GRADE assessment (High / Moderate / Low / Very low).
3. What remains unknown
It outlines areas where research is lacking, where results are inconsistent across studies, whether findings can be applied to specific populations, and what remains unknown regarding long-term outcomes.
4. GRADE recommendation
It summarizes the direction and strength of the recommendation (strong or conditional), the certainty of the evidence, and the balance of benefits and harms. Because this judgment is made by the AI surveying the literature like a narrative review, please use it only as a reference.
5. Reference list
It lists the references in Vancouver style with links to PubMed or DOIs.
Instructions configured in the Gem
I have configured the instructions below based on Retrieval-Augmented Generation (RAG) principles.
From my experience using them, the accuracy of information ranks in the following order:
ChatGPT (Thinking: configurable in the paid version) > Gemini 3 "Thinking / Pro" > Gemini 3 "Fast" > ChatGPT (Instant: the free version, and the paid version may also default to Instant if the mode is not set) > Gemini 3 (without the instructions below)
Feel free to modify and use the instructions below as needed. For those using the paid version of ChatGPT, using Thinking is the optimal choice as of January 2026, though I expect Gemini will also be updated and become easier to use in the near future.
Here is the link to the Gem again. I would love to hear your feedback after trying it.

I also created a custom GPT on ChatGPT using the same prompt. Please give this one a try as well.

I summarized my recent impressions of Gemini and ChatGPT. With both, I increasingly customize them into Gems and GPTs for specific uses rather than using them "as is." GPT-5.2 (limited to thinking/pro!) works much better for my tastes when optimized per task in a GPT using 5,000 characters.
